Agreement to Mediate

In order to get started all mediation clients must sign and return the Agreement to Mediate which you can download here. After you download the document, please do the following:

1. Print the document
2. Enter the address information for you and your husband/wife
3. Each party must sign and date the document in the appropriate area
4. Send or fax the document to the address/phone number on our contact page.
